Pictured Here: Memory Bear
Hand sewn bears are made by Hospice Care of Middletown volunteers with love and compassion from favorite garments worn by the patient. The family provides the garment and hospice provides everything needed to make the ‘Memory Bear’. The Memory Bears are loving and healing and the ‘urge’ to hug the bear can bring incredible support.

At Hospice Care of Middletown, we are committed to providing comprehensive and compassionate care and support to patients and their family and friends who are coping with life-limiting illness in the greater Middletown area.

We are here for you 24 hours a day, 7 days a week.  An interdisciplinary team of skilled and on call nurses, home health aides, social worker, chaplain, bereavement counselor, and trained volunteers work together to anticipate and exceed your expectations of quality care.

You have a choice when it comes to quality hospice services.  For care with a personal touch from your friends and neighbors in the community, there is only one “Hometown Hospice.”  Talk to your trusted healthcare provider today and ask for us by name or call us directly to learn more about how we can bring quality to your loved one’s days.

A Congratulatory Note From Our CEO

Dear Friends, Neighbors, and Colleagues,

Hospice Care of Middletown is delighted to announce that we have once again received a Deficiency Free Survey from the Ohio Department of Health in our most recent survey in April, 2015. ODH surveys us for compliance with over 300 state and federal regulations governing hospice care, and we were found to be in 100% compliance. As the ODH State Survey Director wrote to us, “Deficiency free surveys indicate a significant effort on the part of the administration and staff. It is an accomplishment for which you and all staff at Hospice Care of Middletown can be justifiably proud.”

In addition, for the fourth year in a row since the award’s inception, Hospice Care of Middletown was recently named a 2016 Hospice Honors recipient, a prestigious national award recognizing hospice agencies providing the best patient care as rated by the patient’s caregiver. You can read more about it on our website under our “NEWS & EVENTS”.

These accomplishments are a result of the exceptional care provided by our team of staff and volunteers who serve our community with such dedication. I want to thank each and every member of our team for their caring, hard work, and commitment to the calling of hospice care.
